Bridges Community Health Centre
Bridges Community Health Centre (CHC) is a non-profit, community-governed health care organization with a mandate to provide primary health care, health promotion, illness prevention and community development initiatives in the catchment area of Fort Erie, Port Colborne and Wainfleet (which are located in the beautiful Niagara Region). They have two sites; one in Port Colborne and one in Fort Erie. They are funded by the Hamilton, Niagara, Haldimand, Brant Local Health Integration Network (HNHB LHIN). They are currently looking for a locum on a temporary basis, until they are able to recruit a full-time physician. If you or someone you might know is interested in working with them until they find a full-time physician contact us today!
The following highlights some further details regarding CHC and the position:
- All staff, including physicians, are on a salary. They do not bill OHIP. The current physician hourly rate of pay for which we receive funding from the LHIN is $128.20 per hour;
- They serve individuals with complex health issues who have traditionally experienced barriers to accessing health care;
- They operate using a social determinants of health lens, understanding fully that there are many reasons (income, poverty, housing, employment, education, social isolation, early childhood development, etc.) that have an effect on one’s health and well-being, and treat the whole person;
- Their inter-professional team of healthcare providers includes physicians, nurse practitioners, registered nurses, registered dietitians, social workers, a physiotherapist, kinesiologists, health promoters, diabetes educators, and community health workers who are outreach workers and who assist with system navigation for the clients;
- They also offer a number of health and well-being group programs that are available to the entire community free of charge;
- All staff must be prepared to work out of either site, as the need requires;
- This organization truly operates as a team where everyone’s opinion, skills, abilities and knowledge are valued and appreciated equally. There is no hierarchy between the clinical staff;
- They are very much a client-centred organization.
- They are a paperless organization. In other words, all of the charting, etc. is done through an electronic medical records (EMR) system. Reports, etc. are scanned into the charts. They currently use an EMR called Nightingale on Demand;
- Clinicians work to their full scope of practice. The NPs and RNs work independently in as much as their scope allows.
The physician would be expected to:
- Be licensed to practice medicine in Ontario and be a member in good standing with the College of Physicians of Ontario;
- Work extended hours at least one night per week;
- Provide care for the most medically complex patients within the CHC;
- Have an understanding of the physician role with the CHC model of care;
- Be able to work effectively as a peer within an interdisciplinary team of clinical, allied health and administrative staff;
- Report to the Program Manager and Executive Director;
- Experience working in a community-based primary setting is preferred;
- Experience working with families living with low incomes, seniors, and individuals struggling with mental health and substance abuse are definite assets.
They are looking to fill the position as soon as possible.
